A Primary Care Network (PCN) is like a team of healthcare providers who work together to take care of your health. Imagine that instead of just seeing one doctor, you have access to a group that might include doctors, nurses, pharmacists, and other specialists who collaborate to ensure you get the care you need.
These networks are set up to make healthcare more efficient and easier for patients by sharing resources and expertise. For example, if you have a chronic condition like diabetes, your PCN might include not only your regular doctor but also a nurse who checks in on you, and a pharmacist who makes sure your medications are working well together.
In simple terms, a PCN is a group of health professionals who join forces to provide better and more comprehensive care to their patients, usually within a certain local area.
