Non-urgent advice: BYTES Primary Care Network

Primary Care Networks build on the work already undertaken and the current services offered by GP practices. Working together with other GP practices, local organisations, such as community, mental health, social care, pharmacy, and voluntary services to support the needs of a population that has grown, is living longer, and may need to access local health services more often.

BYTES Primary care network (PCN) is a group of GP practices working together to focus local patient care to patients across Stockton-On-Tees.

BYTES PCN includes:

  • Thornaby and Barwick Medical Group
  • Yarm Medical Practice
  • Eaglescliffe Medical Practice
  • Park Lane Surgery

As a key part of the NHS long-term plan, PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care. PCNs have the potential to benefit patients by offering improved access and extending the range of services available to them, and by helping to integrate primary care with wider health and community services.
